| I request access to anonymized data collected for the
Generation Study 2 (GS2) for the purpose of scientific investigation, teaching or the
planning of clinical research studies and agree to the following terms. |

- The information I provide will be shared with Generation Program data sharing
and leadership team as part of this data use request.
- After completing the information collected in the USC LONI request portal,
the Generation program team will reach out to me within 5 business days with further
application steps.
- Approved requests also require formal execution of a Data Transfer Agreement
before accessing Generation Program study data.
