I request access to de-identified data collected by the Health and Aging Brain Study: Health Disparities (HABS-HD) for the project I will fully describe in my application form. I understand that HABS-HD data may be used only for the purpose of scientific investigation, teaching or the planning of clinical research studies. I understand that I am required to provide a periodic update on my use of HABS-HD data and to submit manuscripts using data from HABS-HD for review. I further agree to the following terms:

- I will receive access to de-identified data and will not attempt to establish the identity of, or attempt to contact any of the HABS-HD subjects.
- I will not further disclose these data beyond the uses outlined in this agreement and my HABS-HD proposal form and understand that redistribution of data (original or derived) in any manner is prohibited.
- I will require anyone on my HABS-HD data proposal who utilizes these data to comply with this data use agreement.
- I will accurately provide the requested information for persons who will use these data and the analyses that are planned using these data.
- I will respond promptly and accurately to annual requests to update this information.
- I will comply with any rules and regulations imposed by my institution and its institutional review board in requesting these data.
- I will include in any papers, abstracts, and presentations (oral and poster) resulting from their use of the HABS-HD De-Identified Data the following Acknowledgement: "Research reported on this publication was supported by the National Institute on Aging of the National Institutes of Health under Award Numbers R01AG054073, R01AG058533, P41EB015922 and U19AG078109. The content is solely the responsibility of the authors and does not necessarily represent the official views of the National Institutes of Health." The full name of the study ("Health and Aging Brain Study: Health Disparities") should be listed in the Methods/Participants section of the manuscript. "HABS-HD study" can be used thereafter;
- If I publish abstracts using data from HABS-HD I agree to cite HABS-HD funding sources as space allows, and acknowledgement of HABS-HD will not be cited in the authorship line of the abstract.
- If I publish manuscripts using data from HABS-HD, I agree to the following:
On the by-line of the manuscript, after the named authors, the Principal Investigator will include the phrase "The Health and Aging Brain Study (HABS-HD) Study Team," with the asterisk referring to the following statement and list of names:
*HABS-HD MPIs: Sid E O'Bryant, Kristine Yaffe, Arthur Toga, Robert Rissman, & Leigh Johnson; and the HABS-HD Investigators: Meredith Braskie, Kevin King, James R Hall, Melissa Petersen, Raymond Palmer, Robert Barber, Yonggang Shi, Fan Zhang, Rajesh Nandy, Roderick McColl, David Mason, Bradley Christian, Nicole Philips, Stephanie Large, Joe Lee, Badri Vardarajan, Monica Rivera Mindt, Amrita Cheema, Lisa Barnes, Mark Mapstone, Annie Cohen, Amy Kind, Ozioma Okonkwo, Raul Vintimilla, Zhengyang Zhou, Michael Donohue, Rema Raman, Matthew Borzage, Michelle Mielke, Beau Ances, Ganesh Babulal, Jorge Llibre-Guerra, Carl Hill and Rocky Vig.
- I agree to submit a draft version of any proposed publication resulting from use of the HABS-HD De-Identified Data to the Data Access and Publications Committee at the HSC Institute for Translational Research. The review is not a scientific review but is intended to ensure that the items in this document are adhered to accordingly. The review will maintain confidentiality and complete its review within 2 weeks.
- I agree to submit an electronic version of any accepted publication resulting from its use of the HABS-HD De-Identified Data to HSC for archiving.
- I agree to deposit any new data generated from the use of HABS-HD raw data, imaging files, or ancillary studies into the HABS-HD master database.
- I will report any use or disclosure of the data not provided for by this Agreement of which I become aware within 15 days of becoming aware of such use or disclosure.

I understand that failure to abide by these guidelines will result in termination of my privileges to access HABS-HD data. |
